After downloading hotfix 00121 and then doing a verify of files, the game seems to have lost connection to my existing definition files. It reset all the graphics and game settings, and, worse, it doesn't find my mpmissions folder. I verified that my regular profile is selected in the upper right corner profile selection. (note that that also lost my profile's default face, for example). I've restarted the game and it lost my profile entirely. I had my main profile and a test profile in the profile selection list, and both are gone. Graphics settings seem to have reset again. If I minimize and reselect the game, it resets graphics again to some huge size where even the exit button is off screen. Serious problem with hotfix 00121!
